Health screening questionnaires must be completed before the start of any personal training sessions or exercise classes.
Payment for programmes and classes must be made upfront and is non refundable except expressly agreed by us on an individual basis.
This statement is for guidance only. It is not a legally binding agreement between you (you) and Elizabeth Atkinson or B Fitness and Wellbeing (we, us) and does not create any obligations which either must meet.
This agreement starts once you are in the studio for indoor sessions or at the agreed meeting place for the outdoor sessions.
– We will not ask you to exercise beyond what you consider to be your own abilities during a session.
– We will make every reasonable effort to make sure that the equipment and environment are in a safe condition for you to use and enjoy.
– We will consider what adjustments would be reasonable to make to allow you to fully access the sessions if you have a disability or health condition with ongoing assessments of your fitness and health.
– If a session is cancelled, we will make every effort to give reasonable notice to those expected and anyone affected will be able to make alternative arrangements.
If you need to change your booked session contact hello@bfitnessandwellbeing.co.uk to discuss this. Once you inform in advance they will email back confirmation that the session has been rebooked. This is at our discretion and under certain circumstances. If possible it will be rescheduled but if no notice is given or no contact is made then the class, place and payment will be forfeit.
By signature of the Health Screening questionnaires (PARQ), online screening submissions, paper versions or Scholarship agreements the participant confirms that they and all persons covered by this agreement approve the above agreement.

As signed on the screening forms by your agreement with this, your photographs or your baby’s photograph or name may be used in promotions for the classes in leaflets or on Social Media. You agree for permission of this unless you opt out in writing.
We may need to collect and process personal information about you including your name, age, address, medical and physical well being and contact details such as your telephone number and e-mail address. If you make block bookings your payment details will be needed to pay into the account. These details will be made available upon request.
We may also ask you for other optional information such as how you heard about the sessions and what sort of fitness areas interest you. If you choose to give this information, it will be used to help provide you with the best possible service and additional sessions according to demand. The collected information will be kept strictly confidential unless specified at the time, and comply with the new GDPR rules and Regulations from May 2018 , meaning the records are stored securely, either manually or electronically, fairly and lawfully processed, and used only for their clearly intended purpose, as stated in the privacy policy. They will be kept for seven years after your last class, unless requested to be destroyed. I have specific details in the record keeping logging payments which will also be securely stored. Your bank details are never stored by me, only by your payment method.

There are many other symptoms and causes of Pelvic Floor Dysfunction and incontinence including neural dysfunction and structural dysfunction such as Prolapse (which can happen to any of the pelvic organs and has varying degrees).
